Health Booklets and Verification of Numbers Form

Dear Parents,

There are still quite a number of pupils who have yet to hand in their health booklets. If you have lost or misplaced the health booklets, please issue a letter with your signature so that it can be shown to the nurses from Health Promotion Board.

Also, hand in the verification of handphone and home numbers form to me through your child as well. If you have lost the form, please e-mail me both yours and your spouse's numbers and the number to receive SMS from the school in case of an emergency. (Only 1 number, it can be either mum's or dad's hp no.)
